Ischemia (insufficient blood supply and oxygen delivery) and other heart diseases may be present in many situations in which chest pain is absent (e.g. silent ischemia) . Furthermore, a significant proportion of heart attacks (myocardial infarction) are also silent or asymptomatic. Many recent studies have shown that the majority of ischemic episodes in patients with symptomatic coronary artery disease are actually not associated with angina.

Finally, the awareness of heart monitoring in public is not enough. Among the deaths from diseases of the heart and congenital malformations that occurred during 1999, 16.5 percent occurred in an emergency department (ED) or were dead on arrival (DOA) and 46.9 percent occurred out-of-hospital*. Nearly half of heart diseased patients aren't aware of their heart status. If one fears their heart status is worse (e.g. after exercise or after watching a horror film), they may go to a hospital or stop current activities to try and prevent further damage.
